Grace Excellency Care is an health care state-licensed agency and is accredited by The Joint Commission. We provide compassionate, quality and affordable health care services to the perform care Children’s System of Care (CSOC) and adult intellectual with developmental disabilities (DDD).

At "Grace Excellency Care" our direct support professionals (DSP) operate according to the preferences of the members and their loved ones. Our professional and exceptional care DSP team are available to introduce your love ones to the community and assist them to be independent. Our DSP are thoroughly screened and trained through the BOGGS CENTER on Developmental Disabilities (Rutgers Robert Wood Johnson Medical School and College of Direct Support)

At Grace Excellency Care, we provide quality services to individuals and groups of our members with intellectual and developmental disabilities. Through these services, we aim to provide our members with the tools that are necessary for their success. We have highly qualified professionals who are trained to provide the highest quality of excellency care.

Our staffs are committed to enriching the lives of our members through support and collaboration to ensure that quality services are delivered efficiently and based on the individual’s needs.

Our DDD services include:

  • RespiteServices provided to Individuals unable to care for themselves that are furnished on a short-term basis because of the absence or need for relief of those persons who normally provide care for the participant. Respite may be delivered in multiple periods of duration such as partial hour, hourly, daily without overnight, or daily with overnight.
  • Individual Supports
    Services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the Individual’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community.
  • Pre-Vocational Training
    Services that provide learning and work experiences, including volunteer work, where the individual can develop general, non-job-task-specific strengths and skills that contribute to employability in paid employment in integrated community settings.
  • Community Inclusion
    Services provided outside of an Individual’s home that support and assist Individuals in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community. Community Inclusion Services are delivered in a group setting not to exceed six individuals
  • Community-Based Supports
    Services that provide direct support and assistance for Individuals, with or without the caregiver present, in or out of the Individual’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan.
  • Career Planning
    Career planning is a person centered, comprehensive employment planning and support service that provides assistance for program Individuals to obtain, maintain or advance in competitive employment or self-employment.
  • Natural Support Training
    Training and counseling services for individuals who provide unpaid support, training, companionship or supervision to Individuals.
  • Supportive Employment Individual
    Activities needed to help an Individual obtain and maintain an individual job in competitive or customized employment, or self -employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities.
  • Supported Employment Group
    Services and training activities provided to Individuals in regular business, industry and community settings for groups of two to eight workers with disabilities.
  • Support Brokerage
    The staff will assist the Individual (or the Individual’s family or representative, as appropriate) in arranging for, directing and managing services.
